Enrollment Requirements

Thank you for your interest in enrolling your student at Chapelwood Elementary. In order to process your child's enrollment in a timely manner, please bring the following documentation with you to enroll. Proof of Residence

Name and Address must appear on at least two of the following:

• Lease Agreement or Purchase Agreement or Mortgage Papers
• Governmental Agency Documents (Example: AFDC Documents, Armed Forces Documents)
• Current Bills (Example: Utility Bills, Charge Cards)
• Current Tax Statements
• Employer Records
• ★ Must be official documentation ★

Immunization Records

Birth Certificate

Social Security Number

• Preferred but not required

Withdrawal Information from Previous School

Examples if available
• Most Recent Report Card
• Test Scores
• Transcripts (Official or Unofficial)
