Instructions for Intention
to Employ/Work Permit

1. Student must be hired first.
2. Employer fills out intention to employ. 
   1. Must be signed by the employer, student and parent.
3. Student and parent go to Ed. Center to get work permits.
   
   1. Take the intent to employ, the student's birth certificate (copy of birth certificate is acceptable).
      
   2. Hours and address for Ed. Center-8:00 am to 4:00 pm 1220 S. High School Road.
